MASUM 2009 - UM Squash Team

It was an exciting week in UUM for MASUM (Majlis Sukan University Malaysia) 2009. Ponteng 1 week class to join the um team for the men squash team event. I'm in specialized surgery posting for this week (neuro, plastic and paed surgery), anyway it's over d, hopefully doesnt affect much lar. This was a great opportunity and experience to go out and join the atlets from other Unis. I'm stepping into final year already, this would be the last chance to paint more colours in my Uni life.

I'm not the greatest squash player in UM, but few of the players were not managed to go, so Ronie and I were recruited to UUM, Kedah to fight for UM. Changlun, a place full of memory, a place where I took my pre-U course (Matriculation) 4 years ago, had changed alot. There is a new shophouses and huge supermarket, which was not exist during my time here. Anyway, it's good to come back here after many years.

UM squash team, which had 2 superb players (Fizul & Asraf), managed to get into semifinal in the tournament. However, we lost to UPM in semifinal with 2-3. It was a tough game, where I as the 5th player, was under great pressure to play the deciding game. The guy from UPM was very good (at least to me), I lost the first game, and beat him back on 2nd game, then he grabbed back the 3rd game, then i managed to win the 4th game to make it 2-2, and sadly on 5th game, I lost to him with 2 points. Just 2 points that we needed to enter Final. Anyway it was a great match. My thigh muscle is still painful after many days now. We beat UIAM and got 3rd at last.

Neurosurgery, going into the brain with IGS (image guided surgery)

Wednesday was Dr. Dharmendra's OT day, we were lucky to have the chance to see the IGS performed on 3 patients. Moreover, three of them were using the different technique of IGS. Basically IGS is a image guided surgery, in which the computer helps the surgeon to identify and localize the lesion in the brain in order to perform some procedures such as biopsy, drainage etc.

Brain is the most vital organ in our body. Any small damage may cause serious impact on the patient function. Therefore, these modern technology help to localize the lesion to even less than 1 mm error, giving a much better operation outcome and reduce morbidity and mortality.

IGS uses the help from CT Scan and MRI, it can combine both images to produce the desire picture. 3 types of IGS are framed, frameless and making a preset pathway. The computer is from BrainLab, currently on trial period, and the technicians were there to explain and show the way to use the computer.

This is the first patient with frame. After CT scan immediate before surgery, the CT picture is transferred to the computer. After setting a preferred pathway to enter the brain for biopsy, the computer will calculate the angles for surgeons. You can see Dr. Dharmendra is setting the angles on the gadgets.

The top and bottom gadgets are set with the same angle to calibrate the accuracy. The needle touching right on the centre showing good accuracy.

After incise through the scalp, drilling is done to go through the skull.

One can see the drilled hole on the skull. Scary isnt it?

The second patient uses frameless technique, not as accurate as the framed one, but still has very high accuracy. Note that there are 'cameras' on the patient to identify the probe location.

The location of probe will be translate into the CT Scan of patient taken earlier. However, this technology doesnt show the insight view after procedure is performed eg. abscess drainage on this patient. Brain structure may change after drainage, thus the image is no longer accurate after that.

Third patient has a preset pathway to let the needle enter. A needle holder which looks like a robot hand, is set up based on the calculated angles. The biopsy needle will then enter the brain, and the computer will show on live of the needle entering.

I'm in neurosurgery, plastic surgery and paediatric surgery for this 2 weeks. They are highly specialized surgical field, and I have no idea what's going on most of the time. But it's nice to have the chance to see how things work nowadays. I believe this opportunity is not available everywhere, as it requires a modern facilities, good technicians, suitable patients and skilled neurosurgeon (there are only very few neurosurgeon consultant in this country!)

Btw, we had lunch in OT (first time) with Dr. Dharmendra, he is very nice and we are glad to learn with him. Going for for MASUM (Majlis Sukan Universiti Malaysia) in UUM Kedah today, playing squash. Gonna back on next Wed or Thurs, will miss quite a few classes. Hopefully I will pick up.

Flying Kites at Kepong

Second time climbing Batu Cave staircase

Last Sunday I had a wonderful birthday with my darling. I went to Batu Cave for the second time, and dropped by a place when on the way back to PJ. It was a famous kite flying place, well known in Kepong. Thanks to TC for introducing the place to me.

Simple but colourful rainbow kite, it's big!

It was located at the side of the highway, you won't miss it if you are driving on the MRR2 highway. There is a sign board stating "Awas! Kawasan Permainan Layang-layang" something like that, just keep left and you will find an entrance to the park. The parking is free and I find many locals went there for jogging and enjoy kites.

Is'nt it easy?

There is a big nice lake in the park with jogging track around it. Running 1 round would already make u sweat profusely. The scenery is beautiful, a good place for recreational activity. Right beside the parking, one can see many beautiful and colourful kites hanging nicely along the pathway to be sold. The kites are not that expensive, a normal simple one would cost about RM3,5,7 depending on the size and design.

We bought a RM5 with rainbow colours :) It's big and easy to fly (according to the Tauke) It was windy and many people were on the spacious grassy field. The kites were very nice, got all kinds of cartoons, spiderman, batman, with special design and shape, looks cool! Flying the kite was not as difficult, just leave it on the air opposing the wind, and slowly release it. The wind was strong and it is very easy!

It was a fun place to hang out, can't believe there is such a relaxing recreational place with traditional activity going on. So many cars were parked beside the highway when we were leaving at 5 something. So plan a weekend there to relax your mind and bring back your childhood memory!

A doctor's writing, can u read?

This is the handwriting seen commonly in the ward. Maybe it's not the worse, but can u read it? I had given up after attempting to imagine what are those 'symbols' representing.. finally end up not knowing what happen to the patient. Ask others? i think the person who himself wrote it might not able to read also haha :)

So, practice good handwriting, at least in a readable way.

Been to some places to eat recently, most of them are nice, but sometimes must watch out the price! Let's start from lower price :)

Currently in surgical posting now, rotating under general surgery. Relaxing? not really.. Have to cover ward (sien..), follow clinic, watch surgery in operating theater, go on-call, attend ward rounds (7am in morning!).. wat else? but it's not as stressful as Paeds :p

How to upgrade your Nokia phone?

I am using a Nokia 3120 classic, and i noticed many discussion about upgrading from firmware 7.16 to 9.41. Due to unknown reason (itchy hand), I tried to upgrade it. Thought of doing it long time ago but since I don't have the USB cable until yesterday when it comes with my father new handset.

First you need to have things below:

Nokia phone (Fully charged)
Nokia USB cable CA-101 (Cannot update via bluetooth, memory card etc.)
Stable and fast internet
Nokia software updater (download at http://www.nokia.com.my/A4500249)

Backup all your data in phone! (Menu>Setting>Sync & backup>Create backup)
Save backup file in your memory card or better still in your computer
Install Nokia software updater (NSU)
Connect your phone to computer via usb cable
Choose PC Suite in phone
Load NSU and follow the instruction to complete

Upgrading is quite an easy and simple procedure, but my phone turn out to be 'taiwan-ised' when I noticed I can't type Chinese input in 简体 and pinyin 拼音. Otherwise it was okay. So I want to share my experience here of how to solve it, i think quite useful for other nokia model users also.

I googled around the net and learned that everyone has its own product code (in my case is 0564330), for the same model, it can have many product codes for different regions and countries. This is because with the product code, the NSU will install the relevant softwares including language package for that region. So in order to get the Chinese pinyin input, I found that the product code 0559100 has it. It has the firmware 9.41 also. By changing the product code, the phone should have the required language.

Warning! This is an invasive procedure, any error might cause damage to your phone. If you are using original nokia phone, better go to the authorised service centre to fix it. Although i doubt they will concern about the language thing.

Now you need another software: Nemesis Service Suite (NSS)

Install NSS in your computer
Connect your phone to computer via the same usb cable.
Open NSS, click the top right button 'scan for new device'
Click 'phone info'
Click 'scan' under the action column
Wait for all the details appear (about half a minute)
Click the 'Read' on the right, the Product Code will appear
Replace the previous product code with the new one (that you have found)
Click 'Write'
Close NSS

Now you can update your phone again using NSU, after finish downloading and installing, your phone is up to date!

Comments mentioned there are some upgrades in the video function, otherwise i have not noticed any major changes so far. So do it only after benefits are ascertained :p

小儿科short case

今天终于来到了，等待了好久，小儿科的最后一个星期，考了临床检查(bedside short case)就无忧无虑了。在马大医院各部门中，对大部分学生来说，小儿科是最‘恐怖’的。八个星期的posting不是开玩笑的。。

今天考short case，早上八点半，我七点就跳起来，不知道还有什么没准备好，Developmental assessment背熟了吗？四大系统（呼吸系统，心脏系统，肠胃系统，神经系统）一直在脑袋徘徊，教授会考些什么怪怪的东东吗？小孩不合作怎办？哭了要怎么哄？

又想到不会回答怎办？搞错诊断，弄错finding，又或者miss掉重要finding，到时真的会死到很难看罗。。

没完没了，最后不想再想了，有太多东西担心啦！到时见机行事吧。。

告诉自己，再恐怖的都经历过，去年在Klang时被出名恐怖的教授拿过，还好当时大步难过，虽然今天的老师还好，但她们也是厉害‘洗人’的。

检查了上战场的武器（儿童玩具，用来检查成长进度的），听筒，两支笔（一个给孩子跟住写），Red pin，测量尺等等，就这样出发了。

虽然只是End of posting test，但是大家都很紧张且害怕。。主要是因为儿科讲师吧。有大半人肚子都不舒服(anxiety 啊),有的手脚发冷，还有朋友不停上厕所，像得了糖尿病似的：）

老师特地从名单最后一个开始，我就成了最后那几个。天啊！等待比想象中痛苦噢。。以前考试我都在蛮前面的耶。没办法，等罗。从八点半等到十一点多，朋友一个一个被叫去，在房内等待的越来越少，大家都不出声。

有些人相当镇定，有些坐立不安，有些还在死命的读，这时可看到各人处理压力的表情罗。我一直努力让自己镇定，不要乱了脚，不然肯定不能做好。

好不容易轮到我了。带我去个七八岁男孩前，当时有点开心：因为第一，不需要做成长测验（变数很大，小孩不合作或忘了部分成长里程碑，都是致命的），第二，不需要做newborn assessment，因为我也没真正做过，惭愧！第三，小孩蛮大了，应该相当合作，躺在那边乖乖让我检查就好了。

结果老师没叫我做任何系统检查，只是叫我看。发现他嘴下方有淤血。就这样进行下去，看了整身，其他部位也有点淤血，蛮大片的，还有些很细的红点，但是很少，还好看到。一面问答一面检查，一路排除几个可能性，最后印象是ITP。

如此另类的Short case出乎我的预料，因为通常都做系统性的检查啊。不过过程需要很多思考和检查技术，Short case的目的依然是一样的。其实还有更多特别例子，比如nutritional assessment，还有去年曾经有朋友只是要求检察头部。如果你没feel到fontanelle稍微full（不是bulging这样明显！），就糟糕啦！

所以说short case可以很恐怖，尤其是在明年Final时。到时又有时间限制，10分钟一个病人（包括检查，报告和讨论），连续做三个病人，包含全部部门（内科，外科，骨科，儿科，妇科等），紧张又刺激，医学生的心脏还不错，能承受考前和考时的压力，要不然早心脏病暴发啦。

看完罗，明天回家休息一下吧！下个星期踏入外科罗(surgery)，也是一个不好搞的posting，surgeons脾气都不怎么好，硬着头皮上吧！

Stop searching for ideal unity

Everyone in UM is racist. We sit in the dining hall according to race, even in lecture hall you can see clumps of people arranged according to race. When the students go out for social activities, they do so in groups according to races. When we room, we room according to race.

Then again, even within the race, there are cliques of best friends. You will notice that when people go for class, it is in groups (unless alone). Even when they sit in lecture hall, it is with their best friends (unless alone). Even when they go out to eat/watch movie/what not, it is still with their bloody best friends. But somehow no one faults them for that. Why? - Because everyone has an understanding that you are part of that clique because you share the same interests and beliefs.

My point is, the whole reason why people go around by race in the first place is because they are more comfortable with people with similar culture and background, just as each race has subgroups with people arranging themselves with people they like, who have the same/complementing personalities, beliefs and goals.

So what is the big deal with division of the races within campus?

People can see that the lecture hall is divided into races, but does no one care to see that we are all in the same lecture hall? Does no one care to see that we sit in the same dining hall and eat in peace without fuss at why another race is sharing the place? And does no one realize we go to class and interact well without getting at each other’s throats everyday?

What we need is a different perspective on unity and racism. Everyone’s talking about unity as if they expect everyone to gel in together as a homogenous mix of people. But in order to have a homogenous mix of people, you need everyone to be (most importantly) of a single culture and a single language. But how are you going to do that when people insist on keeping to (and only to) their own language and culture? I’m not saying that it’s a bad thing. What I’m trying to say is this – If you expect to stick to your own language and culture for most of the things you do (which is natural because we are all most comfortable with the way we were brought up – I repeat, I’m not saying it’s bad), don’t expect an ideal, unrealistic form of unity! Just be thankful we are all here, in the same place coexisting harmoniously, albeit within our own subgroups and race or whatever!

(Note: For those who would like to rebut and do see a group of people of different races hanging out, as in REALLY going out together often, eating and watching movies together, stop and observe – don’t you think they share the same language, culture and mindset? Even groups of mixed races do go out once in a while which I feel is overlooked a lot of the time, because it’s “only” once in a while, which makes it sound more trivial than its significance)

In conclusion, we are only racist because we have a strict definition of the word, blindly adhering to it without taking into account the factors I have mentioned above. I think we should stop sticking to the perfect idea of unity, taking into account how much we love to adhere to our own native languages and cultures, and take on a new concept of unity – living together with respect, in a friendly atmosphere without intentionally condemning or harming each other. Isn’t that unity in itself?

Cheras Pasar Malam 臭豆腐

Went to Cheras pasar malam on Wednesday, a place where was once known to be the longest night market in Malaysia. This is my 2nd time there, since ss2 pasar malam nearby is more than enough for me :) However, there are something more special about this place!

Chau dau fu (臭豆腐), a famous stall here selling this 'smelly' dau fu is the main reason why people travel far to come here. One has to queue long for about 20+ ppl in front of you, but it's worth! The chau dau fu has a bad smell from far (especially when queuing), but when eating it, it is crunchy, only a bit smelly, and in fact taste quite nice, hehe! Although some of u might disagree with me..

There are so many more of delicious food here.. but i have not tried them mostly. Strong recommendation from friends!

All kind of food, clothes, groceries, accessories, shoes etc etc, name it u will find it!

Very crowded..will be sweating even in open space, so wear short pants there :p

这两个星期是Final year学长的大考，这场仗就快要结束了，看见seniors那又紧张又期待的心情，感觉好羡慕哦！过了这关，宣誓礼后，你就是马来亚大学MBBS医学系毕业的医生了，没有人会再质疑你的身份，不再像我们这些Medical students在医院内给病人推三推四，将信将疑的眼神。

这场仗现在看起来容易，转眼就过去了，但背后的心血可不是一朝一夕的。五年的寒窗苦读，为的就是这一刻。好几个月前学长们很多都已经闭门修炼。人家在外看戏，你在读书。人家放长假回家，你还是在宿舍读书。人家在吃Tenji Sushi，你在宿舍打包回房吃。人家在外唱K，你只能在房间听听歌。人家在外到处旅游，游山玩水，你最多周末出去附近走走。。

一年只有农历新年和开斋节假期，其他都是non-stop的。。

或许这不只是他们，而是进入这科系的，一生都是这样忙碌。难道想要成为医生帮助别人，就一定要牺牲自己？每天为病人担忧，那谁来关心你呢？医疗的宗旨，不就是提升Quality of life，那医生的良好生活素质呢？提醒病人早睡早起，足够睡眠，多做运动，自己却晚上没得睡，迟睡早醒，运动或许只是在医院爬楼梯吧！

这些问题好多人都讨论过，报章也看多了，情况是有改善的了。

"You have one life, do something"

Hitz FM常听到这句话，其实说得很对。人生最重要就是活得开心充实，所以热衷你喜欢的，同时别忽略身边亲爱的，同时也为大众服务，合理的牺牲是理所当然的。

明年今日，就轮到我们了，是怎样的心情呢？

毕业了，就是人生的另一个旅程，要面对更多的挑战。学长们，祝你们将来都能实现自己的梦想，把这颗炽热的心继续奋斗，也为我们UM MBBS争一口气！

Btw,考完试放大假耶，到处去游玩去见识吧！也让自己好好休息一下。

玩火自焚

昨天多手罗，去下载Partition Magic来试试，结果整个Drive D不见掉！其实是因为我的C Drive不够位置了，一直出现Low disk space，我用着的是旧型的IDE Hard Disk，所以买新的不值得，再加上我也没用这样多，一买就整两三百GB,超浪费的！一直都用着40GB所以也惯了，要时常“清理门户”罗！

想到个办法，将现有的Drive D清理下，然后把多余的位置让给Drive C，听说Partition Magic可以做到，就放手试了。才刚开始执行，发觉要很多时间，再过一阵姑姑车就要来，想想早停好过迟，就把任务中断。Restart后发现Drive D不见了，还以为照片都抄进家里电脑，以为还好，突然想到里面还有许多重要文件！糟了怎办？

还好Windows仍然能运作，回来KL后就一直试，用了好几种软件，有些看到了失去的文件，但就是Retrieve不到。原本的Drive D已经变成PqRP，partition magic的种类，Windows是无法读取的。在网上看了好多建议，但还是不行。直到深夜，在友人的协助下，终于有个软件能顺利将文件读取出来，谢天谢地啦！要不然我又要从打Case summary，不过也写没两面，但省了很多麻烦罗！

再次感谢友人哦！我也成功把Partition弄好了。学到的教训是：不要随便更改partition！一定要做好backup才可乱来：）

SkyTrex - A physical and mental challenge! Don't miss it!

Yesterday almost 30 of us went to SkyTrex, a flying fox challenge in Taman Pertanian Shah Alam. It's fun to go with lots of friends, coz we were dominating the place! Haha! Situated not far from University Malaya, about 20-30 minutes drive (if no traffic jam), SkyTrex is worth for a visit. RM35 for adult, if more than 10 ppl can get 10% discount. Besides, one can go around the big taman to visit the other places.

Free shuttle bus service in the Taman Pertanian. It's open air~

It's a challenge of physical fitness & toughness, overcome the fear of height, go thru the challenge successfully, and enjoy the game! It's about 1 hour plus for the whole process, providing you are the first timer and no ppl 'taffic jam' in front of u :p

Doing preparation, wondering how to wear the stuff? ask Tatt, he will show u just simple like this!

I would say it's very safe, although we were hanging more than 20m (4-5th floor) above the ground, coz we were checked for the equipments, and had a briefing before start. The guy explain everything and everyone of us had to go thru a smaller challenge to know the correct step. Basically we had a roller to put on the rope, a primary hanger to attach on it, and another rescue hanger to lock on rope. So everytime when moving from a place to another, there will be at least a safety lock on u.

But anyhow, if u happen to detach both of them from u, there is absolutely no place to hold in the platform, falling down from 5 storey height is not joking :p The challenge was not as easy as thought, climbing 5 storey height on a 90 degree stair was not easy even for a fit guy in one shot. Some of the challenges were not easily done, if you cant make it, there will always an alternative (coz you are on safety attachment, also a 'chicken exit' for those who fail to move on).

Anyway, all of us manage to get thru these, and had a fun time there! We still able to take many photos on such height, risking the camera just to take down the valuable moments!

I forgot since when I started to have the feel to submit some 'creative' photos to The Star for the thumbnail competition. Why? simply because the winner can get RM50! Taking note of some interesting stuffs around me is my special skill. (not to say i'm ke poh lar :p)

Each day 5 photos will be selected and one of it will be the winner. It isnt that difficult to get your photo selected, coz I submitted 4 photos so far, and 3 of them were published! But sadly.. none of them was the winner :(

This is my first published photo which was taken when I was in Klang doing practical (around early 2008). My friends and I were on the way to Kuala Selangor to see the 'kelip-kelip' and eat seafood. The car was hanging there after an accident. Since I was holding a camera (prepared for the kelip-kelip), I zoomed and snapped the picture. I am lazy to find back the link from The Star coz it was so loong already..

There was another photo submitted after this, but it was a bit sensored because it involved bugs doing something sensitive. I think that's the main reason it was not selected. I couldn't find it in my laptop to share it here. But I think it is the most difficult shot to take among the four. I will post it when i found it :)

Second published picture (5/2/2009). I was in the bus no.12 heading towards University Hospital when the bus engine started to burn. We ran down after shouting from the driver. It was lucky that all passengers were safe in the event.

搭渡轮回家真好！

新年后都没有回到家，五个星期过去了，对有些朋友可能根本不是什么，对有些人可就好久罗！我觉得呢？还好吧！自从在吉打念matric开始就习惯在外过生活了。但是有一样东西始终不变，就是那美好的回家旅途：）

想起以前要从Changlung（靠近泰国边界）搭kereta sapu到Jitra，因为那边才有长途巴士站。还记得每次都乘搭Transnational，十五分钟一辆non-stop，不选它还选谁？

到了北海码头，家乡的气味就浓厚罗！看着海洋对面那美丽的岛屿，有山有海有高楼大厦，一个充满生气，热情，没KL这样紧凑的城市，Penang我回来啦！

昨天傍晚拍下的，好可爱的一朵云，像什么呢？

昨天乘搭友人的车从KL回来，然后搭渡轮过海哦。渡轮服务已起价RM1.20一段时间，服务有进步了。等待的座位是全新的，像NBA篮球场旁黑人坐的那种，好大张哦！船也没以前等的这样久，有布条写着他们获得什么ISO认可的耶。

熟悉的Ferry，有人选择坐，也有人从头站到尾，欣赏“无敌海景”啊！

我常光临的面包档，满好吃的，价钱合理，通常到这时又是肚子打鼓的时间，不用考虑啦。

可能最近在儿科部门压力大，有得回家松懈一下，感觉又充满活力了。只可惜每次回家，我的老朋友+好朋友都不在，很难跟他们timing的啦，一个在东一个在西，乖乖在家罗!

Jom Futsal!

We did not have another indoor futsal game since the last time played during Klang time, which was one year ago d..

So, it's time to gather everyone to have fun!

15 of us, from boys to girls, from white to dark skin (ronie lolz) went there in the morning. Surprisingly, we were the first team who arrived. KL ppl really wakes up late..

There are 4 big courts and 2 small one. It is well maintained with nice artificial grassy carpet. The building is high and open-air at all sides, so it is cool even in the afternoon. Student price is available at daytime, even on sat n sun, at RM40 for small court while RM50 for the larger one.

It's situated right beside Summit Shopping Mall, not hard to look for it.

There was a furniture shop beside it too. Large parking space which is free.

It's nice to play indoor futsal once in a while coz we can shoot as many as we can! The passion of scoring a goal is different from playing in college's court where one has to search for the ball when it flies over the fence :p

We played for 2 hours.. kinda tired but it was great!

Tiring Paeds Posting..

I am in paediatric posting now for 2 days, and it was full of work and stress. My group was assigned to paediatric ward 1, which is the intensive care unit. Besides have to cover the bed in the ward, we still need to clerk more patients to present. Some more there are tutorials, lectures and on calls..

Just came back from on-call, where have to follow the round at 4.30pm till dinner time, then another round starting at 8pm. Students have to cover the new admission before 8pm and presented to the lecturer during the round.

Some more i like know nothing although went through paediatric in klang for 2 months.. everything seems so difficult and not as easy as i thought. I had been alerting from early morning till late night, luckily i had a sleep in afternoon, coz tuesday got lecture which allowed us to disappear from the ward, for few hours.

Having a bit flu now, must take care of health in order to keep fighting on. Paeds is known to be the busiest n stress-est posting. I wish to look at it differently, and i will work twice or even trice harder, to conquer my first major posting!

Batu Caves - A visit

Today was a tiring but meaningful day, because I have visited to FRIM and Batu Caves! It was organized by my friend TC, special thanks to him again. This was my second time to FRIM, so took the canopy walk again :p

Batu Caves was my first time there. It was a sunny hot day, but we were still inspired to move on after lunch. It is just minutes away from FRIM, if coming from KL via LDP, it is about 15-20 minutes if no jam AT ALL. hehe!

From outside can see a huge statue in bright golden colour, very impressive!

The long and steep stairs is the challenge, but wont be a problem for normal person.

Huge Ovarian CA from theatre 卵巢肿瘤-手术室

A major surgery in operating theatre (OT) for gyne was done few days ago. The patient is a chinese lady in 40's, noted abdominal swelling 3 months ago.

Never see a huge tumour? this is the process of removal of tumour mass in OT. This is a major surgery which may have severe blood loss and complications.

After opening the abdomen, a huge ovarian mass was found

The plan was as follow:

TAHBSO (Total abdominal hysterectomy and salpingo-ophorectomy) - Remove the uterus and both ovaries and fallopian tubes, together with the tumour.

Appendectomy, omentectomy and lymph nodes clearance. - Remove the appendix, the omentum (the 'policeman' which attached to intestine) and the lymph nodes around the pelvis. This is to prevent spread of the cancer.

The uterus, tubes and ovaries were removed. Note the huge ovarian mass on the left. It had both solid and cystic component, suggestive of malignant carcinoma.

Weekend walk-away to Cameron and Pangkor!

It is so hot in these days in KL, it would be nice to walk away from this sauna city to a natural, beautiful with cool and fresh air place, the cameron highlands. Moreover it was Valentine's Day on last saturday, can buy nice and fresh flowers there at much cheaper price. I went there before several times, but it was long long time ago.

4 of us went there by car and stayed in guest house. The air was cold and refreshing! But the sun was still strong and burning in the afternoon, luckily we brought enough umbrella to walk around. We tried some food there and they were generally nice, including western food in T-Cafe, Nyonya food in Ferm restaurant, Steamboat in Hong Kong Restaurant and other local food in coffee shops. They were nice and not as expensive as i thought, they were more or less the same price in KL.

It was a nice trip, cameron highland worth for a visit. Never been there before? It's time for cuti-cuti Malaysia! It's best to drive there, then u can have more freedom to go around. Btw, the old road was really pening and difficult to drive, slow and long. If you are going from north eg Penang, use the new road from simpang pulai. We took the old road from Tapah coz from KL.

Ong Bak 2 & 海角七号 Cape No.7

Recently i watched some movies and would like to share here:

1. Ong Bak 2 - Although I did not watch the first episode, i decided to watch after much good recommendations from friends. The movie was nice, with variety of cool martial art, Thai's culture and history. Thumb's up! Dun miss it, if u like action movie, worth to watch in theatre for best effect.

2. 海角七号 Cape No.7 - A famous movie from Taiwan, it's not new and i got it from other :) The first part was kinda boring, but it became more n more interesting after that. It is a nice romantic movie, with great scenery (that's why putting the pic on top hehe) and lovely songs. Despite that, it's meaningful also :p.

I'm going for a trip with my dear n friends at Cameron Highlands and Pangkor island on tomorrow till Sunday. Hopefully it will be a nice and memorable trip. Still having a bit URTI, 'bottom-less' running nose now lolz, not sure what will happen in the cold weather of Cameron Highlands.

Will share some photos n stories here when i back :) I never go to Pankor before, i'm going to one more place in my cuti-cuti Malaysia list :p

坐落在SS2附近，不知叫Section几了，明天饮食中心，一个很大很多东西吃的地方！来过几次了，太多档了，每次都尝试不同的食物，专找些特别点的来try。

前两天回来KL时搭巴士，Metrobus 的 12 号巴士要回马大医院。上桥时听到有点怪怪的声音，还以为是哪个乘客在搞怪，怎知巴士后面引擎突然发出很响的吵声，司机赶紧把巴士紧急停在路旁，并喊"Turun!Semua turun sekarang!"

大家都吓倒啦，争先恐后要挤出车门，车后冒出浓浓的烟，好像随时会爆炸！好恐怖啊，我又挤在后面，那时感觉生命是脆弱的，一旦发生灾难真是不堪设想。

还好大家都顺利跑到路边，离得巴士远远的，车后冒出的烟掩盖了整条路，后面的车在不知前面发生什么情况下也不敢前进。

巴士引擎似乎不能关掉，引擎吵杂声一直响着。车坏在马大KL门附近，我懒得等下辆来打救，就走路回啦！身上大包小包的都是新年食物，好重哦！走着走着又下起雨啦，没办法只好冒着雨走上斜坡，好累。。还好朋友车经过，载我一程。

无论如何，能逃过一劫，已经感到很庆幸，没人知道那巴士接下来会发生什么事啊！走得好好的竟然会这样，是不是没定期检查？唉，没事就好！

Happy Mobile is not that happy anymore

I was clicking thru the net these days looking for a suitable talk plan operator, and i discovered that the Happy Operator (my previous talk plan number) had changed its wonderful rate into a lousy one..

Before this, it was RM0.99 to ALL operators, including Maxis, Celcom, Digi, home fixed lines anywhere in M'sia including Sabah n Sarawak, for max 45mins. This is the most valuable plan for ppl who chat long.

Now, read the following latest updates:

"During Happy Hour, an extra long call up to 45 minutes at anytime from 9:00am – 5:00pm will only cost an affordable RM0.99!

Calling your friends will always be easy on your wallet even if they are on other networks! Enjoy long conversations at only RM0.99 for the first 15 minutes, and RM0.33 a minute thereafter to all networks, nationwide. If your call is under 3 minutes, pay only RM0.33 a minute for all calls to your Happy friends.

新春大团圆

年除夕，午餐与家人去吃点特别的，好久没吃的Pizza。现在有新年特别的Golden Happiness Pizza，中间有虾，旁边有充满Cheese的‘黄金桶’，味道不错。

家里有好多新年饼和糕，今年少买了，但是亲朋戚友给了又很多罗！难怪新年没机会饿啊 :p

婆婆不在后，我们依然每年在三叔家吃火锅。拜了祖先后才开动哦！

A Glimpse of Cytology Lab

Few days ago we had a class in pathology department regrading histopathology of O&G specimen. I'm currently in O&G now, but in the new system, we only taking Gynecology for 4 weeks now. Obstetric will be continued during the major posting rotation.

Pathology is a great branch in hospital, where they in charge of most specimen for investigative and diagnostic purpose. Specimens are sent here and the pathologists have to handle hundreds of specimens within a day in this busy hospital.

The machine on the left is used to make the specimen onto a single layer cell, sounds unbelievable? it also can reproduce more sample for other investigation.

Place for staining, looks familiar in our laboratory in second year.

In O&G, PAP Smear (for cervical CA), endometrial sampling and high vaginal swab are some of the common specimens done. They are usually done in the outpatient clinic and the patients are asked to review the result on next appointment. Using a Cusco's speculum is not as easy as i thought, as young patient is not as 'lax' as the older or multiparous women.

Sunway 重庆麻辣火锅！跟整班钟灵buddy去吃，不错哦！特别的是把薄薄的肉片放进麻辣汤烫下就吃，特别特别。。对面就是出名的缘火锅（以前介绍过），这里没这样拥挤，也没抢得这样凶。火锅多人热闹吃得就开心罗！

Red Box 唱K 午餐配套，牛肉啐spaghetti，蛮香的，又大碟，真是吃喝玩乐哦！去这个时段最值得啦，十一点多去可唱到三点噢，只要歌曲继续播，还可唱更旧呢！

靠近马大医院，往Jalan Gasing路边，叫怡保芽菜鸡的，也很好吃。店面很大，员工都用PDA点菜，可能因为这样，价钱蛮贵的。还是马六甲的中华鸡饭粒更香好吃又便宜。

熟悉吗？马大生应该对Section 17的麻辣板面不陌生吧！吃了好多次，还是一流。它是在出名的糖水档旁边，个人觉得干捞比较好吃（上图），可能不太能吃辣吧！:P

SS2 Murni的Sizzling Yee Mee。我也不知道为什么会点这道，平时没叫过就试试看罗！味道普通，而且蛮咸，还是叫些热门的较有水准。

汽车看病记

上星期六拿车去修理，因为最近车无端端死火，所以就拿去检查看下，一下是病历报告：

Chief Complaint: The car had few episodes of mati engine for the past 1 week

History of presenting illness: Occured a few times, worse on yesterday, relieved by restarting engine after few times.

Past Medical History: Long history of air cond maintenance, engine always had leaking oil in the past.

Past Surgical History: Changed tyres and water hose last year.

Family History: Many protons suffered from this problem.

Drug History: Usually takes Mobil octane 92. Good compliance :)

Social History: Has a good owner, which is me, wahahaha! Currently the 2nd hand user.

Examination: The wire for engine might be loosened, which causes the mati engine problem. It was fixed and should be reviewed in few weeks time. However, there was accidental finding of leaking engine oil. With experience, it was believed that the meter belt and the rubber stuffs were spoiled. Need immediate surgery!!

So I had no choice but to fix the problem, which cost RM250 including labour. Plus RM50 for changing gasket of engine.

Haih.. spend alot these days, i dun mean to waste money unnecessaily, but things just come one by one out of my control..

维修车子就像人看病一样，唯一不一样的，是车子坏了可买过，人的生命失去就救不回了。修理车子真贵，尤其是在KL做，但没办法啊！难道驾回槟城做？半路坏了到时花的可不只三位数罗。。最近电话和钱不见，损失惨重，还要修车，新年又要到了，真对不起父母，当初该保管好自己的东西。

仔细想想，病人看病，同样也面对金钱的困难，但身体的健康是最重要，更没得选择放弃。所以医生们啊，对经济困难的病人可否减价呢？

修车的都没得扣，何况是专业的医疗？世界就是向钱看齐了。。

How to split myself into 2?

Fortunately squash we managed to get into semifinal, and volleyball into quarterfinal. But a problem arised now.. next match for both games are on next Monday same time! So how? I need to go set the balls in volleyball, squash we still do not know who is the opponent, it is better every player is there for strategic game..

But i think i will make volleyball as priority coz squash can play earlier or play the last one. We have good players in the team, 3 um players, excluding me :) i'm just another supporting player, haha!

Wish to get some medals this year. Got some skin abrasion in volleyball match, i want something rewarding to pay back!

Sports Week!

This week is sport week for me. I joined sukmum (sukan mahasiswa um) in volleyball and squash. Yesterday we won the first match in squash, and the next match is on thursday. For volleyball, tomorrow is the first match in small group, against 4th college. We have quite a few good players and hopefully can win some medals back.

Going to be a busy and tiring week. Luckily i'm still in primary care for the last week. But still hvnt done my case summary...

Feeling a bit lazy these days, watching prison break and it is great! Scoldfied really has a superb mind.

A day with Hospis Malaysia

Friday we went to Hospis Malaysia in Cheras. It was part of the PCM posting attachment. Hospis is actually similar to palliative care of terminally ill patients. They go to visit the patient's house to give proper care. Many of these patients are cancer patients who had poor prognosis. They suffer from pain and many family and social problems. The staffness will go and assess them, review the condition and medications, do examination, ask for complaints and make proper management.

The nurses report to the doctor every morning during meeting. The doctor will review every case and make adjustment as required. So the nurses will follow the advices and prescription. Sometimes, the nurses can also adjust the dosage or change medication with sufficient experience. We followed from the morning meeting, till separated into small groups of 1 or 2 persons to follow the visit. There are about 7-8 nurses in charge of this home visit. They have their own patients to follow up, and received new patients from time to time.

During visit to a patient, saw these houses with so called 'cartoon house'. Heard that it was once a very expensive house sometime long ago. It is situated beside highway, looks nice from far.

Hospis Malaysia is a NGO where it is supported by the community. They rely on donation and sponsorship from companies. Patients are therefore do not require to pay for the service. However there are many patients who volunteerily to donate due to the kind service provided.

There was a conference in UMMC not long ago regarding palliative care. The health ministry is working towards giving a better palliative care not at terminal stage, but to introduce it at the beginning of disease. They are getting support from many parties include hospital oncology department, palliative care units and Hospis Malaysia. In future, not only these terminally ill patients will be taken care of, but other types of diseases will be given proper care during the process of treatment to reduce suffering.

Visit to spastic centre

Today my subgroup for primary care medicine went to spastic centre near University Malaya. This visit is to give us the opportunity to have an insight of this health and educational facility.

The school was well equipped with many facilities. There were old and new buildings, which consisted of various level of classrooms, vocational classrooms, music room, physiotherapy, occupational therapy, speech therapy, hydrotherapy (mini swimming pool) etc.

The children there were very friendly and cheerful. We were brought by the physiotherapist around the centre, and then free to go around the place. Playing with the children was fun :)

We should not show pity to them, but to give love and courage, and thus they will go further.

Lost my phones n money..

Something horrible happened today (sunday), my phones n money were stolen.. i lost them when i went to wash clothes for a while. My fren came to my room also, the time gap of no-man period was so short. I discovered the lost soon i came back room. Luckily that guy didnt take away my wallet, only the money inside..also my 2 phones.

My room was not locked but closed with the lock hanging. Maybe i should lock whenever i go out including every toilet visits, but thats something troublesome too.. i believe in the security here, maybe it is only for residents in college, coz that guy in cctv recording was coming from outside with a car.

Report had been done in the campus security. But i think nothing much can be done, beside a small hope to trace back the car plate number. Anyway, things that lost hardly comes back.. miss my sony ericsson phone alot..

ID card flashing for the gates in all building entrance should be started 2 weeks ago, but it was delayed. If it was started, outsiders definitely can not come in and walk out that easy. Residents of university hostel should have the right to stay in a safe compound.

Be careful of ur belongings, there are so many bad people outside, they will be punished by the God one day..

New Year Eve with Fireworks, Justin 侧田 & Ip Man 葉問

Goodbye 2008, say hello to 2009! an exciting new year eve at one utama, kl. Before this we were thinking of buying some goods and watch a movie there. Luckily we saw brochure about new year concert at the central park, with the arrivial of Justin 侧田, famous singer from Hong Kong, what a surprised!

The central mall of One Utama was nicely decorated with Christmas and New Year's theme. Look at the bright shining christmass trees on the stage, and the cards design on the floor and around the hall, must have spent alot on these.

The movie, Ip Man, was really as good as others' comments, should hv watch it earlier! The movie greatly showed the spirit of a man, and the community, in fighting for dignity and their future. The Kung Fu was impressive, fast and cool, together with the art and moral behind every punch. Dun miss it! It is still widely shown on all theatres :p

Finally, the concert! We went there after the movie and a bit of shopping, coz the artists at the final one hour is always the best, haha! Managed to squeeze a place near the stage, but it was not in front of the stage, but behind, lolz!

To highlight here, is the fantastic fireworks! The fireworks style was following the rhythm of songs, nicely done with many climax. It lasted for about 10-15 minutes, 3 songs with different feel. Another was the arrival of Justin at 12am! Right after the fireworks, he sang for 5 songs, including his famous and new songs, so generous :)

Leaving after his performance, haha, the traffic was not as bad as expectated. Polices were checking on the highwat when going back, probably checking back seat passengers for not wearing seat belt. So remember to wear, coz it is 2009 now!

藥劑師根本就不獲重視。這就是為甚麼許多華裔藥劑師都在與政府的4年服務合約屆滿後都不願留在政府醫院。被醫生無理取鬧的罵，甚至被“老油條”的護士罵和病人罵。幾個星期前，她因看不懂醫生寫甚麼字，一橫畫過去就是藥名，打電話去詢問那醫生，那名醫生二話不說就破口大罵。她還面對上司的種族偏見和無禮對待。。。

陳君不懂程序，難道陳君的女友也不懂嗎？身為一個專業人士兼大學畢業生，如果這種普通常識也不懂的話，我為本地大學栽培出這種藥劑師感到悲哀。這種“以為”的想法也是本地大學生只會死讀書的最佳證明。。。

事實往往不是這樣。北馬的人，卻被派到南馬；南馬人，卻派去北馬。更甚者，還有為數不少的東馬人拿著數目可觀的外州津貼在西馬各大城市就職，反之每年卻有無數西馬人被派到東馬。這樣的調動，試問可有任何標準、制度可言？藥劑師往往連申請的門兒都沒有。醫生卻被強制遷入醫院宿舍。有不少配予醫生護士的醫院宿舍是轉租承讓給無處蔽身的藥劑師。。。

醫生、護士和藥劑師肯定有差別，醫生、護士有宿舍是因為要值夜班。在大學沒租過房子嗎？出來工作還要替他大小姐準備宿舍？還沒工作便耍大牌，這算甚麼態度？被醫生和護士罵有甚麼大不了？沒有工作經驗，罵罵兩下不就有經驗了嗎？在外工作可不是享清福，沒讀過比爾．蓋茨的書嗎？比爾老兄說要多吃苦才能成功，挨老闆罵沒甚麼。。

雖然配藥看起來很普通，很簡單的一份工作，可是我們接觸的都是病人的生命，畢竟不能拿病人的生命開玩笑。就拿個例子來說，一個24小時沒睡覺的醫生，你放心讓他替你看病、開藥方、甚至動手術嗎？我們藥劑師也一樣，你放心讓一個24小時沒睡覺的藥劑師替你檢查藥單、幫你配藥嗎？

衛生部的政策是避免醫務人員長時間工作，他們必須獲得足夠的休息，因此我已經指示所有醫院，必須在值夜班過後讓待命醫務人員適當休假，以讓他們補充精神。我們強調，衛生部並沒有忽略藥劑師的問題，我們持續改善藥劑師的福利。衛生部已經調整了藥劑師的薪金制，使他們享有更好的待遇，例如新進藥劑師已經從P1T1，調整至P1T4。隨著這項調整加上津貼，使得政府醫院藥劑師職位更具吸引力。

其實牙醫的待遇並沒有亦凡君所說得那麼理想，事實上是跟藥劑師一樣，同樣要守著U41直到服務期滿為止。醫生目前的遷升路線，首兩年的實習期為U41，後三年的服務期為U44。而牙醫雖然同樣也有分第一年的實習期，和後兩年的服務期，但卻一直都是停留在U41，並沒有遷升至U44的機會。。。

實習醫生一週至少會輪值一次晚班，而該晚班是緊接著白天的巡房工作開始的，每一次值晚班之後，醫生就得開始隔天的日常巡房工作，也就是說連續不斷的工作 36個小時對醫生來說根本就是家常便飯。雖然說當值的晚班醫生有oncall room可以休息，但試問究竟有多少的時間能讓醫生好好的坐下來合一下眼？

如果不跟有動手術的外科醫生相比，一般上牙醫的染病風險，肯定比普通門診醫生還要來得高出許多。醫生、牙醫和藥劑師，雖然這三個科系至今都還是馬來西亞政府大學的前三甲熱門科系，可是如今有亦凡君，筆者以及陳君在此申訴藥劑師、牙醫和醫生的無奈，實為無病呻吟。最無奈的，也許是正在閱讀著我們文章的病人吧！

也因為是在大醫院，人事關係也更為複雜，尤其是那令人生厭的階級制度。許多實習醫生都被大、中醫生視為下人，不斷得被施於最勞累的工作。尤有甚者，被護士或護士長欺負的事也時有所聞，就因為他們是最低下的菜鳥。資源有限，僧多粥少，滿足不了所有人。不必比較誰更苦楚，其實大家都是同一條船的人。不如互相扶持、同心協力，聚合起來才有力量。我們國家的決策人若不儘早處理這些問題，更多的無奈還會發生，更多的人才還會流失。。。

什么是PCM?

什么是PCM？不是paracetamol（俗称panadol）哦！在这指的是Primary Care Medicine。

在马大医院常听到人提起去RUKA看病，其实是马来文翻译，全名是Rawatan Utama & Klinik Am，英文的Primary Care变成Rawatan Utama，服了吗？呵呵！

刚进PCM(Primary care medicine)一周，其实只上了三天，过后就自己放假了:) 在Klang时已经上了四周的PCM(Klinik Kesihatan Bukit Kuda)，在Final year时将会再到Banting的诊所进行课程。所以接触时间也算不少。

PCM就是在外面clinic的百科医生，他们都叫General Practitioner(GP)，毕业做完实习医生，再跟政府服务完，就可到外头当GP。在医院就称Primary Care，老师常说，他们是医院的把门员(gate keeper)，除了常看些头痛发烧咳嗽之外，最重要是提早发现严重问题，懂得何时把何人refer去相关部门，愈早发现治疗效果就越好。

同时，病人的教育也相当重要。Ideally，医生该花些时间跟病人沟通，灌输有关方面的知识，除了可达到预防效果之外，病人在了解病情的根本之后，才会信赖医生，会依照指示定时吃药等，减少non-compliance（没依照指示吃药）的问题，达到最好效果。

现实是残酷的，在Bukit Kuda时，医生看病只有少过五分钟，房间拥挤吵喳，根本没有时间和私人空间跟病人好好谈。而且很多时候病人来是因为depression或anxiety，往往主诉是其他问题如头痛背痛等，所以需要时间让病人把问题说出来。

不是所有门诊都需开药方，
吃药也未必是最好的疗方，
越少药方就越少副作用，
没有药方的治疗，用得正确，病人会安全噢！

这里介绍一下我们在PCM看病时应有的策略,叫做Murtagh's Strategy:

Probable Diagnosis （最可能的诊断，通常是常见的问题如伤寒）
Important/Serious diagnosis not to be missed （不可错过且严重的，如肺结核，癌症，心脏病等）
Frequent pitfalls （常被忽略的，多是较罕见的问题如infectious mononucleosis,leptopspirosis）
Masquerades - Depression, Diabetes, Drugs, Spinal dysfunction, Anxiety, Thyroid abnormalities, Urinary tract infection (3D SATU) （要scan过这些可能性）
Hidden agenda (病人有潜在的问题，如家庭纠纷，索取MC等）

有了这个策略，就能做出更准确的判断。

然后要以Patient centered的approach，做出一个Whole person diagnosis，也就是以病人的人（不是对病disease centered），做出全方位的诊断：

Physical
Psychological
Social
Behavioural
Effect on function

如此就能对病人在各方面的影响进行探索，在处理(manage)病人时有极大的帮助。

最后在病人处理方面，可以Scott & Davis model来manage：

A - Acute management （如止痛，退烧，给antibiotic）
B - Behaviour alteration （如停止抽烟）
C - Chronic problem （如糖尿病，高血压）
D - Disease Prevention （如皮肤敏感要少吃海鲜类）

PCM的医生，肩负着重大的责任站在最前线，不该被金钱利益蒙盖了眼，一切决定都应以病人的利益为出发点，这样就可大大减少antibiotic resistance,昂贵医疗费，门诊商业化等问题。

Jusco Member Day

Yesterday was the Jusco member day at the new Aeon Jusco at Bukit Mertajam. In KL, it is a grand shopping day for the ppl coz all brands will mark down the price, 70% are everywhere, or even more.

In Midvalley, the Jusco used to only permit 2 person per card to enter, ppl waiting to enter in early morning, grabbing the clothes like craze, queue long for trying, it is the day for the J members. With RM100 spent, can get RM10 voucher, sounds nice isnt it?

But these were sometime ago...

We arrived in the late afternoon, it was not as many ppl as expected. I went staright to do a member card RM12 on the spot, because afraid of long queue later. After shopping and purchasing, only i realised everyone can go in to enjoy the discount, not only for member, zzz...

The voucher redemption for member is only RM5 per RM100 expenditure (General merchandise, excluding food). I think this is the new rate, will be the same in all Jusco.

Anyway, the items were all marked down at lower-than usual price. The hottest spot was the ladies handbags. So many 70%, and in the centre there was a happy hour event sale, discount at 80%! Someone was using loud speaker to shout about the super sale, really very local (haha!) like in market..bet u wont see this in KL or in Penang Queensbay Jusco..

But it worked! many ppl outside rushing in to grab and choose, the bags were selling fast, with new stocks kept throwing into the war zone.

I bought some clothes and other things, all at least 50% :) In the shoe department, I asked the promoter about the price, some shoe like camel active are 30% during this Xmas n New Year sales, yesterday was increased to 50%. So if u are planning to get one, wait for this time!

But it was not as crowded in KL, maybe it is new in BM Penang, but i think it is mostly due to the economical crisis, everyone is saving hard for their pocket. So, only buy something if needed, dun get influenced by the discounts. I always remind myself about this, but still... hehe :p

Once per year, in each local Jusco, see u next year!

Merry X'mas & Happy New Year!

This is a pic taken not long ago at the new Aeon Jusco at Penang. The place was nicely decorated with X'mas & New Year atmosphere. Walking in the building reminds me of Klang Aeon Jusco, because the design, the shops, the cinema, they are all the same! Secret recipe, then got Black Canyon opposite the Big Apple donuts, looks so Klanged, miss the days at Klang oo..

Sales were everywhere, and the Jusco Member day is on this Friday! (tomorrow). Perhaps i will go to look for my stuff coz the discount is much more, and i believe Jusco in Bukit Mertajam wont be as crowded as KL kua (need to take long queue jz to test a cloth!).

Again, we went to Big Canyon to eat, the place where we (with classmates) used to celebrate Dr. Chuah (Paediatric lecturer) farewell. The food there was nice with its famous coffee, leaving me good impression. There was some kind of promotion set so was nice!

Actually i went there last weekend, then back KL for few days, and came back yesterday with Tatt's car. Sounds free huh? haha! Dont want to miss this festival season mah! Lagi i'm in Primary Care Medicine posting now, so i'm relatively free :)

Having a bit running nose n post-nasal drip causing a bit sore throat, hopefully it is abit n will recover very soon, cause i'm going for buffet lunch with family later at Golden Sand Resort. We went there long time ago when i was small, n my parents like it cause the food very nice and varied. Hopefully the award-winning restaurant will colour the day of Christmas!

The Return: New Komtar, Pacific

Today I went to new Komtar with family, to have a look of the newly opened Pacific supermarket in Komtar, Georegetown. Coming back to this place was full of memory.

Looking 10-15 years back, my parents used to bring us here to shopping in the weekends. During that time, Komtar was the most happening place, crowded, full of people everywhere. Throughout the years, Komtar became quiet, worsened and finally stopped business for many years. This was mostly due to the opening of new shopping places like Gurney Plaza, Queensbay, Tesco etc.

Now the new Komtar comes back, opens right before the festival season and chinese new year, can it shines bright like before? we will see :)

国产车，亲情，知足，快乐！

国产车新的Proton Saga在路上越来越多，我朋友就有好多个家里都买了辆，我家也跟进‘大队’，排了好多个月才拿到车。妈妈在今年四月底订，原本说三个多月就可拿车，结果等了半年才踏进我家，可见反应热烈哦！我朋友AK家在五月酱订，我去台湾前问他（那时十一月），还在等呢！

款式新颖，潮流感，马力小，最重要价钱不贵，是它的卖点哦！不是什么有钱人，有辆代步工具，坐得舒适，省油，麻烦少，那已经是满足了。什么八秒半到达100km/hr，什么超大cc马力，钱太多不如去做慈善哦！

最近看了部钱不够用2，相信很多人都看过了吧。当人到山穷水尽时，守护在你身边的才是最爱你的人。人心不如蛇吞象，人起了贪念，虽然物质上可能比人富有，但失去的可是无价的亲情，友情和爱情。有颗知足的心，生命会更灿烂！

浪漫一定要驾豪华车，用烛光晚餐，在KL Tower看烟花赏月吗？一起等巴士搭地铁，一起寻找道地的大排挡，一起在溪边海边谈天说地，无拘无束的，也很开心的哦！

当人有了太多的方便，就会失去过程的宝贵经验，那是生命的一点一滴，是一起吃苦的幸福啊！

不知怎么本来想到国产车的，却说到这里来，呵呵！夜深了较多感触吧。。

这使我想起了几年前一段Petronas的广告，我看了很多次，那信息还是那么强，感触还是那么深。这里和大家再回味一下吧！

婆婆的儿子驾着国产车进来那一刻，永远留在我脑海中，
知足常乐，孝亲敬老，无论将来如何，希望都能铭记我心。。

钱是万能的吗？我始终都不曾同意过。

醫協：2015年醫生過剩

（吉隆坡）大馬醫藥協會指出，大馬將在2015年以後，出現醫生供過於求的現象，因此有意攻讀醫科的學生必須做好應對壓力的心理準備。http://www.sinchew.com.my/node/94772?tid=1

目前大馬醫藥理事會需要處理至少5宗新晉醫生面對壓力、情緒及行為改變的問題；一些新醫生則無法應付長時間的工作。

Doctors now so cham have to work so much till get psy.. when the houseman are enough in future, i believe the problem will be lesser.

However, by looking at the statistic, when my batch graduate at 2010, there is still lack of 9000 doctors, but the Dr:Patient ratio has improved from 1:1236 to 1:747, hopefully we will have a not-that-stress working load when come out.

Btw, publishing this statistic on papers, is more of telling students not to simply choose medicine lo.. an improvement in medical service & quality, but a much more competitive medical life in future, good news or bad news? i dono..

周杰伦的中学，[不能说的秘密]拍摄点-淡江中学！（淡水）

前天是在台湾最后一天罗，我们早上去了故宫后，就打算去淡水。台湾MRT捷运非常方便，几乎台北的所有旅游景点都可由捷运到达，或在捷运站坐个公车就可到达，旅游手册都解释的相当清楚。

淡水是红线捷运终站。我们乘搭公车去渔人码头的方向，沿途有经过真理大学（要问问才懂淡江中学下这站），下车后就往斜坡上走。斜坡路口是间什么红毛纪念馆的，像是古时西方人占领时留下的建筑物。没多余时间，我们赶紧向上走，一路上好多像学生又向旅客的年轻人，情侣也不少哦！

经过的建筑物，有点像戏中的建筑物，所以拍了下来。

经过真理大学，不远就到达了‘私立淡江高级中学’。名字有点夸张，中学还有分超级不超级的？哈哈！不过应该是这间吧！问一问守卫，他很明白的就说旅客可进入，看来已有很多人慕名而来过了。

开始经过一些古老建筑物，它是所基督教建立的学校，包括淡江中学和真理大学。然后有几座新校舍，最后来到旧校舍。我们一到就有‘感觉’了，似曾相识的感觉，好像电影内的某个角落！我觉得走廊有点像，但还是不肯定，最后问了那里的学生，才知道在不远处，取景多在课室旁的走廊。

这是学生说的走廊，戏中男女主角在此相见的哦！

这里也有点像。。

不过他们说不是所有景点都在此校，比如戏中的琴房等都是在别处，有幕黄秋生在草场旁与女主角对话，他们说是在真理大学草场，于是我们过去看了下，但觉得不很像，反而中学草场有点feel，大家看看吧！

Finally i'm back!

After a one week trip around Taiwan, i'm back to my hometown again :)

Taiwan is a great place to go, we able to go quite alot of places within one week, with minimal expenditure.

For many days i hvnt update my blog, so thinking of 'summarizing' my 'diary' here..lolz! So here is the trip n places we went:

Friday: Finish half day class at TsuChi, took train to BanCiao(板桥，near Taipei) stay one night. (plus Stephan n SuHuan)

Sat: Taipei 101, 西门町

Sun: Took train to Jiayi (嘉义）then took bus to 阿里山. The special train to 阿里山 stopped service, no chance to experience it. 阿里山森林游乐区. Stay one night. (student package)

Mon: 玉山看日出. Too bad the sunrise wasnt the perfect one, coz the sun was blocked by cloud. The place was very cold, nearly 0 celcius, luckily we wore few layers :p. Aftern back to 嘉义 and straight to Kao-Hsiung. Went to 六合夜市.

Tues: Kao-Hsiung （高雄）- 佛光山，旗津，西子湾（爱河），六合夜市, then took high speed rail (高铁）back to BanCiao

Wed: Taipei (台北) - 二二八纪念馆(meaningful), 总统府(place where president works), 中正纪念堂

Thurs: Taipei Zoo, 国父纪念馆

Fri: 故宫(nice, many historical things to learn n see)，淡水(went to JayChow's secondary school where the movie '不能说的秘密' was taken, also went to 渔人码头, a nice seaside with great scenery). At night went to 士林夜市, it was superb crowded!

Sat: which is yesterday, took early flight back to M'sia, arrived in late aftern. Came back to Penang at night.

Before this during the 3 weeks in HuaLien, we went to places like Taroko national park 太鲁阁, 慕谷慕鱼，鲤鱼池，七星潭，宜兰，焦溪泡温泉，慈济精舍etc. These are place where one can go during the attachment time in Tsu Chi Hospital in HuaLien.

Take note that if possible dun buy things at 7-eleven coz they are expensive! haha!

其实是。。。

Mitsubishi Lancer!这是旧款的Lancer，Wira买它造型，所以非常相似。台湾没国产车，到处都是进口车，以日本车如Toyota,Mazda,Nissan居多。路上到处都是豪华房车，连德士都Vios或Camry，难怪台湾在个人机车排烟量冠全球第三！

想要靠近它吗？

All about Taiwan! 宜蘭溫泉

I hvnt got the chance to online until today. Last week was busy for chinese medicine test (simple n cincai..haha) then saturday went to YiLan and Sunday went to Jing She & attended a chinese medicine course at hospital.

YiLan is a nice place. Famous for Hot Spring water, traditional cultural centre and night market. Besides that, also got wine factory n museum, lots of local food, new shopping complex, prison and cultural village. Lets share some photos here while i can explain a bit about the places :)

This is called Rou Geng Fan. The pork is made into small pieces, abit like meatballs but is handmade, taste delicious, recommended!

Outside cultural village, tell the history of YiLan, the culture & tradition of the people.

Look at the scooters here! The transparent plastic fixed on it is used to prevent cold air from blowing directly on the face. There is even wiper fixed on it, very cute :p

Many people enjoying hot spring water at the public hot spring garden. This site is strictly for leg only! you can jump into it if u want to :)

Night market at Luo Dong. So many stalls to eat and many things to buy n play.. some stalls hv to queue for very long to buy it.

Once arrived at the Jiao Xi station, can see many banners promoting Hot Spring water.

Street of the cultural centre, each building has its unique tradition. Spend more time here as the place is so huge, and its getting dark at 5 smth!

I'm doing my last week attachment is Tsu Chi, which is internal medicine in cardiology. Quite free here actually, as told by senior, but if want to attend all classed, then it is different story :) Learned about threadmill jz now from Dr Liu, a nice physician whois knowlegable. We will start our 'trip' after Friday. Probably going Alisan, Kao Hsiung and Taipei :)

中医部，进来参观哦！

这个星期在中医部，基本上就是在中医的门诊,算是医院内门诊人数最高的其中之一。在这几天我们学了也见识了各种中医疗法，其中包括针灸，推拿，各中草药等等。另个朋友上星期已在这了，所以这星期他带我们学习。

我还是第一次真正接触到中医，在这是中西合拼，效果很不错哦！这里的医师都有西医的qualification,有些已经是西医专科，再读中医，住院医师都有硕士资格，所以能处理更多的问题。他们也接受来自病房的referral，所以会到病房替病人针灸调理。

很多中风人士在这都能获得相当好的进展。但是穴道好多，各种草药与用法更是多到。。。真是佩服那些医师们。不过回想起来，我们西医也不知不觉学了很多新词，所谓common things come commonly，同样的道理，常见的问题的处理方法就变得熟悉了，对我们这些新手很多时候就‘听没路’罗！

We decided to go Toroko National Park on saturday morning by following the Taiwan Bus Tour. It is located in Hua Lien, but is not reach-able by cycling. We got cheaper price using Youth Travel Card. The morning weather looked a bit dark, but it turned skyblue when the van moving away from the town.

The driver (also the tour guide) was very good and professional. We were fortunate to be in a small group of 11 ppl, so everything was fast and efficient. The driver, Mr. Wong whois a teacher, kept joking and introducing along the way with full of interest. He knows alot about the place, although i know he d repeated it for hundred times :)

There were many spots within the national park, but we were brought to some of the best sites which cannot be missed :)

and the place is very nice! Cool weather, not cloudy, mountains which cannot see the top, the sound of waterfall along the river, the tress, flowers, the famous stones... cant describe in words, come let see some pics!

We were glad that the drive stop by at 七星潭 before back to town. This also a nice place to go, it is not a lake, dun get misunderstand! it is actually a seaside, facing Pacific ocean. The waves are big, strictly no swimming! The scenery around was very nice, not far can see mountains jz beside the sea, and the beautiful gardens jz beside the seaside with bicycle track.

The special thing is the stones. The beach has no fine sands, but full of all kinds of stones, which make HuaLien Taiwan to be one of few famous stones production in the world. Some names that i remember include 大理石，玫瑰石，猫眼石。。

Days at Taiwan!~ 台湾花莲慈济综合医院，大学与花莲市

I was quite busy after the arrival at Taiwan on last Saturday. We stayed one night at Taoyuan Tsu Chi after getting down from the China airline plane. On early morning of Sunday, we traveled via train to Hua Lien. We were given 2 rooms at the hostel, but it is not within the Hospital compound but outside the hospital and campus, beside the main road.

Our rooms at 5th floor, so hv to climb stairs like hiking everyday. Luckily we dun have to walk around like first few days here, after getting a bicycle from Tsu Chi education department. Cycling around here is nice, but not when the weather is freezing cold.

The weather here is getting colder for the past few days, but today is very nice, not cloudy with sunshine, temperature around 22-25C, like daytime in Genting.

Today i went to watch movie with a few friends at Midvalley. It was only 12 somthing after lunch, we drive there and the cars were getting more n more when entering the Midvalley city from federal highway.

All the sign boards with car park zones and availability showed full house! Zone A,B,C... and also the Garden parking. After turning a corner, Zon G & H showed green light, i quickly followed the directions and i noticed the cars around were moving at the same direction too.

The traffic was heavy, moving slowly (luckily still moving!). So my frens went down to buy tickets first. When arrived at the Zon G,H entrance, it was closed with full parking sign board. Oh my god... now where to go?

There was still more than one hour before the movie starts, so i kept trying and going around the place, hoping for any miracle, but the ONLY place available was the Garden Premier car park. That is very expensive and totally not worth it. I guess only ppl with Benz or BMW will go in..

So many cars around me hungering for jz a small parking lot, i'm jz wasting time n petrol there. Finally i gave up and drive back, but my frens said they already bought the tickets...what to do?

Haha! Luckily i know a place to park at the apartment beside Station University. I parked my car there and took RapidKL back to Midvalley. Funny isnt it? But that was the reality in KL, u hv no choice but to turn back..

With only RM1, u can go to anywhere in the region with RapidKL for the whole day. It is really a good and affordable facility, but of coz driving car is more comfortable (but not during traffic jam like that).

2 days later i will be taking china airline to Taiwan for attachment at Tsu Chi HQ Hospital at Hua Lian. So i'm hanging around in KL these few days, n quite free actually.

After a briefing by seniors who went there b4, i realized that there are so many nice places in taiwan to see n visit. Compared to them, we have shorter time to travel around coz we gonna spend 3 weeks on attachement and 1 week to travel in taiwan. So from their experience, we probably gonna go to places where highly recommended, so will be more selective :p

According to them, some places that are 'must go' include Gao Xiong, Ali Shan, Qi Xing Tan and Taipei. (I not really know how to spell or write the real name haha!) During the time in Hua Lian, there are already many places to go around in the weekends and free time during weekdays. We gonna borrow bicycle and explore around ourselves. The good thing in taiwan is that bicycle is safe and popular in smaller town like in Hua Lian.

Taiwan is really an environmental friendly place, besides cycling, they discourage the use of plaster bags, extra charges will be made if u need plastic bags in supermarket. Eating in Tsu Chi cafe is also a good example. We use own food container called '环保饭盒’ to eat or 'dabao', no use of polyester products.

We are lucky to get accomodation in hostel of the hospital. The facilities are great and convenient according to previous batch. The only thing is that we need to come back by 11 or 12am coz the gate will close, so 'gonna sleep early wake up early' （早睡早起）n be a good boy lo haha :) heard there is no wifi access in the hostel, so hv to go to the library to online. Anyway, i will bring my laptop along for my elective work, also for entertainment purpose :p

According to Shiun Woei, the food there is very nice. The cafe only serves vege food. We need to buy prepaid card to eat, but it is very cheap, only about RM2 per meal after converting to ringgit. Accomodation is free & we only nid to pay for water & electrical supply. Feeling very grateful for the good offer from Tsu Chi to medical students like us.

I still not very sure the best way of connection in Taiwan. For phone call we probably will get a new local number for the ease of contacting each other. IDD card is suitable for calling back Malaysia coz its more expensive if use my own maxis number. However, i still activate my international roaming, so only call me if emergency ya! ( i will be charged with same rate when receiving)

I found back my old Skype account luckily. So if the speed allowed, its the cheapest way to call back, but need stable internet connection lor. I will try to update wats going on in Taiwan in my blog, and hope to share more with u all here :) If u hv gone to taiwan b4, u are very welcome to give some advices and suggestions about where to go, what to buy, what to eat, what to play!

Hiking in Penang

Yesterday i went to hiking in my hometown Penang island's mountain. I used to followed my parents to hike in saturdays. So when i back to home i always bring my sport shoes for hiking. I think there is a bit special about penang hiking places.

In penang there are many routes to hike and they are crossed and connected to each other, so one can have varity of choices as each path has its own characteristics. Some are steeper, some are cooler with more trees, some are easier but longer, and some are fast but challenging! so u may choose according to ur taste.

Secondly there are many rest stations in betweem or at the end of the path. Many of them have nice view of scenery, and there are coffee, tea, biscuits and sometimes even fried mihun provided. All food and drinks are free, and one can make donation at the box. Many ppl hike in the weekends, and my parents make many new friends there, this is a good and healthy way of widening the social relationship.

This is the time when my SE walkman phone plays a vital role!

Popular paths to start point include Moongate, inside youth park and botanical garden(inside and beside). They can reach many rest stations such as:

No.3 - coffee, tea, biscuits, fried mihun
No.5 - coffee, tea, biscuits, gym equipments by MPPP
No.38 - coffee, tea
No. 46 - coffee, tea, platform for taichi
84 - coffee, tea, biscuits
Penang hill - many stalls, got one ais-kacang best (cheaper for hiking ppl)

Getting tired sooner than b4 when hiking ystdy, looks like i need more exercise to train back my stamina, now keep eating too much at home and supper in college d :)

前天跟俐莹跑步后不知去哪里吃，突然想起去试试Seapark的Nasi Lemak Panas. 其实已经听说很久了，但是就是没机会去吃，况且我蛮少去Supper的。

印象中它是路边mamak档来，那时才傍晚六点，我们就碰碰运气看开了没罗。老天不负苦心人，我们到时刚开哦！难得天气好，桌子整齐的摆在路边，只是条小路没什么车经过，感觉这地方不错哦。

介绍美食哦，Ho Jia （好吃）！

叫了出名的Nasi Lemak Ayam，还没什么人所以一下就来罗。看起来好好吃哦，果然是'Panas'！饭是烫的，鸡刚炸好，蛋刚煎好，加哩热乎乎的，只有timun和江鱼仔是normal temperature，小心烫嘴哦！

吃起来不错啦，地方广阔，是个宵夜的好去处。一碟RM4.50，现在KL mamak可不是便宜的哦!吃宵夜分分钟还贵过正餐耶！突然想起SS2 Murni也有Nasi Lemak Ayam，但蛮久没叫了，忘了味道如何，不能做比较。

它是在Seapark Shell油站后面，在一间咖啡店旁边，那时咖啡店已关，只剩下它和一档肉骨茶在店旁，周围路旁的桌椅都是mamak的哦。仔细看发觉原来有档名的耶，叫PJ Ibumie。

有机会去尝尝吧！不错哦，只是好像没荧幕，晚上不能看足球罗！不过也好，通常有球赛时食客坐了就不走人，这样就不必争位子啦。

PJ KL这一带还有很多不错的好‘吃’处，但小心不要迷路哦，随便叫个UM或UTAR生应该都很熟悉的。

Transnational vs Konsortium, who is the winner?

Finally for the first time i decided to try transnational express bus from KL back to Penang today. This is referring to the article i discussed earlier about problems with Konsortium bus. There are a few good things to highlight here about Transnational:

1. The information counter has a large board displaying all destination, time, bus plate number, on time or delayed, and is it boarding now. It is updated every second and passengers dun hv to keep asking about which bus and where to wait (as in Konsortium)

2. This is impressive! 4pm bus ticket, the bus departed at 4pm sharp. 3 transnational buses departed together at the same time, showing how punctual are they. The buses were already waiting at the lane b4 the departure time.

3. The bus is very new, 3 seats, comfortable, clean. Drivers and staffs no chit-chatting or making noise around like in other bus sometimes.

4. Bus moved in fast speed. Powerful engine coz it is new, with auto gear (i can feel its smoothness when changing gear).

5. Only stopped once at Plus rest station. Duration was short and no delaying.

6. This is safe! The seats are equipped with seat belt. But none of the passengers used it, including me, haha :)

7. 4pm bus, i arrived Penang island at 8.20pm (hv to count in the time the bus went to Duta station to take more passengers). it is much faster and well scheduled than konsortium or other buses, where more than 6 hours is not something shocking at all (late depature, stop many times & long duration of stop, super slow driving)

8. Ticketing counters available in UM (my school lar university malaya) or even in UH (university hospital).

However, there are a few inconvenience that i found out:

The seat is not as spacious as Konsortium. Transnational has 11 rows, 3 seats per row, making it 11x3=33 seats, compared to Konsortium with 9/10 rows.
The bus will go to Duta station to take more passengers.
The time KL-PG-KL is not that frequent.

So whois better?

I like the good system, management, transparency, punctuality, discipline, and attitude of Transnational bus company.

I like the comfortable seats, frequent schedule, and direct PG-KL-PG route of Konsortium.

Maybe this is why ppl always say "there is nothing perfect in this world"..

Both of them have their pros n cons. Overall, i'm more satisfied with transnational bus as the seat and extra duta stoppage doesnt really matter for me. So for the time being, i think KL-PG with Transnational, and PG-KL with Konsortium, is a better idea.

Sick with trojans..

I have been struggling through for the past few days due to this trojan (malware problem).. my laptop slowly became slow in loading especially during surfing net. The Mozilla took ages to open, and the softwares and games took 10X than normal speed.

When ur laptop is not functioning properly, and some more u need to use it, then life become miserable.

We have a wide local network in college hostel, about 6-7 hubs connecting over 20 people in guys hostel. I'm the lucky guy coz my room can connect to internet thru wifi. So i share the internet to all ppl, and i think the problem actually came from here.

The trojan was detected from my antivirus Kaspersky 7.0 (Licensed wan okay!) It was something like Trojan backdoor win 32... I cant cure it from the suggestion in internet by cancelling it in the registry. Although it reported that it had been disinfected, whenever i deal with local network things like playing network games or chatting (we use BorgChat to communicate, a cool and free software!), the laptop became laggy..

I formatted twice in the past 2 days. After the first time the problem seemed like still persisted. But thinking back now maybe d2 crashing between Kaspersky and ZoneAlarm (a firewall security software), coz Kaspersky prompted that ZoneAlarm was contraindicated in latter time.

So i format again and everything is working fine, until now. Heard that Kaspersky Internet Security provides the best security, but at a higher price. For ppl like us who play Dota frequently, the firewall is open all the time, its at risk of getting infections.

Statistically, some friends formatted computer recently d2 similar problem. Many frens' thumbdrives which connects to my laptop showed similar infection, something like Worm.Win32.Autorun. So beware of these transmitted agent, scan b4 u open the pendrives!

Beware of these viruses and worms, computer needs a healthy body check-up too!

缘火锅，你吃过吗？

前几天整班朋友去火锅,在Sunway的，相信在KL的人都知道吧！如果不知道让我推荐一下:)

什么这样特别呢？其实很多朋友去多次乐都说还好啦，吃多当然也会显。。它有样最特别的东西，就是。。。鸡翅膀！当然不是自己烤啦，现成的，热乎乎，招牌菜哦！

这就是招牌菜鸡翅膀罗！

去了几次都是这样恐怖，每当鸡翅膀一拿出来，就会引起一阵轰动，你会看到一大堆人冲上去，为的就是抢给自己桌。所以每桌都会有些“代表选手”去“竞赛”！

届时会听到欢呼声，也有一大群人脸臭臭走回去，身材大点的就有优势啦！像我这样斯文的真的很难找吃，哈哈！

鸡翅膀每十五分钟出一次，十秒内一扫而空！这店老板肯定赚到傻，三成楼肯定都满座，外头还有大把人在排队，一人二十块，谁说经济不景气？

槟城也有间叫金火锅的也是很不错哦！我比较喜欢那边，选择更多，价钱也便宜，也不用争先恐后！或许，争回来的，所以更好吃吧？哈哈。。

Tears from a mother

There was one Chinese girl presented with jaundice for 2 months. She had episodes of seizure and psychosis recently.. the 13 years old chinese gal very kelian, the whole family n relatives were there when we came to the bed, and she looked dizzy and talking to herself. The parents were so worried after went thru hospital Columbia and Gleneagles without any improvement, she was first thought of having hepatitis.

Many investigations were done on her b4, finally she was diagnosed with SLE and referred to UMMC. i believe she must have suffered a lot all this while. With the specialists available here i hope she could recover fast.

"Dr, please give her the best treatment available, I just want her to be cured, no matter how much the cost will be.."

First sentence that came from the mother, with watery eyes looking at all the doctors surrounding the bed. Suddenly i felt very touched, the love from parents, the tiredness and anxious-looking on their face, after going thru all of these.. if i'm the one lying on bed, i believe my parents and family will say the same words, spending all their time, energy and effort just for me.

It is so fortunate that we have family and friends with us, the loves and courage are sometimes just what the patient hope for, and they might even better than any medicine!

ALTHOUGH you will see smiley and relieved faces on patients in hospital who finally treated correctly, you will never know how much disappointment and anxiety they have gone thru before arriving at the safe platform.

What we see in tertiary hospital is just the tip of the iceberg. When the disease is recognized and diagnosed correctly, most of the time the patient is able to progress well and recovered. However, there are so many cases outside in the society that remained undiagnosed. People are suffering everyday simply d2 poor health care services. Misdiagnosed and undiagnosed cases are everywhere…

Who to blame?

People who continue suffer will blame the attending doctors, which usually the first line general practitioner in clinics, and finally lost faith on health care services.

I used to listen to complaints from patients in Klang general hospital regarding the poor quality of service, and this dissatisfaction occurred on so many patients.

“Private is better than GH..” this is what has created among Malaysia ppl's mind, and something to think of, if there is still hope in our public health services.

A 'Rheumatoid' ward round

I am hanging around in UM these days until next Friday (Argh, jz reminded that i hvnt bought my Transnational ticket back!), and i only need to go Dermatology clinic on Tues and Fri morning to hunt for patients with psoriasis, this is part of my elective project to collect data from them in the form of interview with questionnaire.

Rheumatology (风湿病学) = mainly deal with clinical problems involving joints, soft tissues and allied conditions of connective tissues. The term rheumatology originates from the Greek word rheuma, meaning "that which flows as a river or stream" and the suffix -ology, meaning "the study of." abstracted from Wikipedia (many students find it very powerful!)

Actually i'm under Dr Amir, the only one UM rheumatologist available now :p ... Why am i wondering around in dermatology clinic?

"Incidence of Psoriatic Arthritis among Patients with Psoriasis in Dermatology Clinic"

Yesterday was totally free, so I decided to follow Dr Amir in his round in the morning. Although there is a special ward/cubicle for rheumato patients in Ward 8TD, but many other patients are all over the hospital eg. geriatric, paediatric etc.. now i know why Dr is so fit, coz we keep climbing stairs and walk around like visiting UMMC :)

First case was a middle age man presented with prox muscle weakness and dysphagia. Dr showed us a few signs which are Gottron's papules (pic on the left), Mechanic's hands... sweat! luckily i got bring oxford handbook and flip through.. it is Dermatomyositis! First time saw it, I could bet it is not easily diagnosed coz the signs were not obvious, with only a few manifestation..

Second case was a young lady with SLE (Systemic Lupus Erythematosus) with atypical presentation, no butterfly rash, photosensitivity but the ANA & anti-dsDNA were positive. She got APLS (antiphospolipid syndrome) too.

There were 2 more SLE cases, one with jaundice and another one presented with a rare association, Dr said it is urticarial vasculitis. Urticaria-like lesions were seen on almost every parts of body (pic on right is an example of urticaria). There was also a chinese guy with pleural effusion and pericardial effusion. Water inside the heart is an emergency! But why there is water there? they are still working on it... Bilateral gouty arthritis on both legs was the next patient problem. He responded well to colchicine, and will be discharged once he could walk well. But his presentation was quite atypical too, coz usually it only involves one side.

Looks like many things are not that straight forward, sometimes totally memorize and rely on textbook is not the best approach, it is the experience that we need to collect along the way.

For some time i have been using this 2 softwares in my phone, they are Oxford English Dictionary and Oxford Medical Dictionary. I found them somewhere in the internet quite some time ago. They proved to be extremely useful for student like me, who needs help in some English words, and more importantly, medical words and drugs details!

Oxford English Dictionary - Contains any words u can find n think of, with very good and detail explanation. For beginner and also pro users :p

Oxford Medical Dictionary - This is fantastic! Any medical jargons, terms, syndromes, rare diseases, and even drugs! It never fails me :) The drugs even has its functions, uses, side effects, trade name etc. Very useful to refer, and during questions in ward round!

'Pek Chek' with express bus

"Konsortium is my airasia.." heard this from my fren not too long ago to describe how fast and convenient the express bus from kl to penang. (never take airasia, but its the economiest airline in m'sia)

Do u think so? i dun agree with that, mayb it was good that time..

Taking express bus does not give much good experience to me, too many problems and uncomfortable, i have no choice but to take it..sigh. Recently the price raised for 30% since Raya till now, even in normal weekdays. RM35 PG-KL, that was Super VVIP price in the past. Not to mention the price (since petrol n diesel cost are sky high), the service of this company disappointed me many times

Never punctual during departure
The staffs at station with poor attitude (Ask which bus never answer!)
Poor directive for passenger (that bus going where?wat time?)
Drivers chatting loudly and smoking
Malfunction chair, hv to keep straight or flat the whole journey!
Stop at rest tol too long!
Recently the bus will drive into Taiping to refill diesel, got once it took 30 mins!
Stop 2-3 times at least
Drive slowly sometimes (I dun ask for speeding, but driving at 80km/hr is not express at all)
The last 2 times i took were KL back to Butterworth, 2.30pm bus reached jetty at 8.30pm, 6 hours! (my family were waiting me for dinner, and i still need to take ferry!)
My juniors who took Transnational at 3pm on the same day, arrived even earlier at 7 smth.

Poor public transport service causing ppl to abandon it, if my saga is not that old, i would rather drive back if there are enough people to share. PG-KL still not so bad, KL-Butterworth really disappointing. I asked the driver that day about entering Taiping to refill, he replied it is company policy, what to do?

I seldom take Transnational, but there are counters in UM and University Hospital. I dun have to go or ask frens to buy tickets from Pudu earlier with this good accessibility available. So I think i will give a try on transnational next week when back, AND we will see any improvement or not.

Transnational has RM0.10 extra charge for insurance, kinda weird :p

Another thing i dislike is, the condition in pudu station. So many foreigners, so many ppl shouting, the smokey air, the narrow space, and the high risk of stealing! Not to mention the traffic condition there during peak season, you'd better go earlier if u dun wan to waste ur Airasia!

Why S'pore has excellent public transport? not we dun have the facilities, but its about the attitude, the education, the system, lack of improvement and competition.

Starting my 2 months elective

STILL, many ppl dun understand our system, maybe we are the 1st batch to go thru the new syllabus as guinea pig :p

Ppl always ask,"what year are u now? last year?"

I think 4th year and 5th year is kinda overlap little bit. In the new program of 4th year, we are divided into 3 big groups, rotating between:

1. 4 minor postings (A&E,ENT,EYE,ANAES) - 8 weeks
2. Elective - 8 weeks
3. PCM & O&G - 8 weeks

After that will continue with 5 major postings - Paeds, Medicine, Surgery, Orthopedic, PCM & O&G. This will continue till 2010, ending with my MBBS Final exam.

I have gone thru 4 minor postings, and all of them are cool! They are relatively more relaxing lar compared to major postings. Now stepping on elective period lor.

Basically now is holiday for me..hehe! For this 3 weeks i gonna do research in UH about psoriatic arthritis, together with chen yit. On the 4th week, I will go Taiwan for a month lor! Gonna do attachment at Tzu Chi Hospital in Hwa Lian, Taiwan.

Tomorrow I'm gonna back to KL to do the study lor, but i believe most of the time is still free..wondering what to do leh? coz i only need to present at the dermatology clinic for tues and fri morning. Btw, my topic is about the incidence of psoriatic arthritis among psoriatic patient in dermato clinic.

Thanks to Chern Ling of reminding me about the video.. through out these years i had a few chances of designing some video for certain funtions. They are not of those professional clips, but the content inside tells better of what happened these years.. After many times of pc format and file transfer, luckily some of these videos are still with me. By uploading here at least i still have my very last copy if i'm under virus or trojan attack!

My first video was created during form5 school assembly, kinda crazy doing such movie clip in front of students and teachers! haha..but its very funny and i believe its the memory of our very own batch of frens :)

March 2003-Combined movie clips to give passion and excitement. First time doing so kinda noob :) it was shown during school assembly, and i was pleased with the good response, at least from students..hehe (teachers might not think so :p)

Jan 2006-Malam Kebudayaan (Cultural Night). The video clip was designed and shown during the opening of Malam Kebudayaan held in 6th college, University Malaya. I love this clip coz it comprises of multi-races, cultures, and the cool ending with Lord of the rings & Narnia, to show the 'Power of Unity'. The background songs are nice, so keep ur ears clear :)

August 2006-6th College Merdeka week (minggu kemerdekaan) video. Taken in 6th college to show the students creativity and effort in decorating the college compound, also to be contested as part of the competition among all colleges in UM. I was the cameraman using the college old DV cam lolz!

May 2007-MUNUS Sport Games. MUNUS(Malaya University and National University of Singapore) is an exciting sports carnival between the 2 Uni's medical students. It is held once every year taking turns either in UM or NUS. The video was shown during the closing ceremony to show some photos flashback and result of each competitions. Ending with pirates of carribean (hot that time :>) and prize giving, to UM!

March 2008-CRP (Community Residency Program). This is a project for all 3rd year Medical students with the purpose of learning research and studies, also exposure to the rural areas. We were separated to 5 districts in Pahang. My group was in Raub and it was a great memory! The video shows the activities and places we visited that time. Shown in closing in district office and review in faculty.

Apart from these, there are still some videos created for other events but i d lost it, they are War Disted created for my fren chenn yih, the opening video during Kedah Matric Chinese New Year celebration and the video for graduation day in secondary school (i think it was distributed in VCD). I guess CRP is my last time having the chance to play around with the audio and visual stuffs, busy for study and lots of other stuffs, my lovely video software 'Pinnacle' also duno thrown where d.

Hello friends! I'm Chuan Huan, currently a 4th year medical student from University Malaya.

I am inspired to create a blog (my very first one..so kinda noob) after exposing to my senior who is working now in hospital. Telling and sharing my stories with others is something new and fresh for me, but i feel it is a very good way to share and keep contact and keep updated with my friends.

I not sure how people can get here, but writing something here keep me remind of what i have been gone through so far. 'a picture tells a thousand words'..rite?

I always wish to share those exciting moment with others, recording down myself as a precious memory. But to start holding a pen and write on a paper sounds stupid for me..haha :) I have to admit that the advance in technology allow more people to communicate. Writing in front of laptop is simply more comfortable for me, and i love the audio n visual effect and interaction in it.

Watch I'm not stupid 2 before? The blog of the boy reflects its importance now :)

SO.. i'm just another medical student who is struggling for studying-life. What else to say? suddenly i feel to much to tell, too much to say, coz every moment i went thru was full of memory..

Starting as a chubby boy in primary school, trained into a dark volleyball player (n fit! hehe), involved in many activities in secondary school, organizing events in matriculation, and extending my life in UM medical school. Many happiness, sadness, passion, touching n love in each and every part of them!
